I don't get what a Fill in actually is. If you have one measure 4/4 do you add the fill for an example in the last quarter note or do you add the fill in after you have played the measure through and then start a new measure?

Fills normally start either at the beginning at the bar or halfway through and finish on beat 1 of the next bar. Fills are used to introduce a change in the music ie. to go from verse to chorus
